AP 413 - Medical Documentation
Background
Horizon School Division No. 205 is committed to ensuring the health and safety of its students, staff, and community. Horizon School Division recognizes and accepts the responsibility to provide its employees with a safe, healthy, and productive work environment. Employees have a responsibility to report to work, capable of performing their tasks productively and safely.
Horizon School Division shall, when required, request medical documentation of its staff as outlined in this procedure.
Purpose
The purpose of this Administrative Procedure is to establish the Division’s expectations for medical documentation and employee absence management processes, appropriate behaviour, to provide consistent guidelines for all employees, and to clearly communicate to employees who are unable to perform their regular duties and must be absent from work.
Procedures
1. The Director (or designate) may request a medical certificate from an employee at any time.
2. An employee who is absent due to illness for fewer than 5 consecutive working days may be required to submit a medical certificate verifying their inability and/or restrictions to work.
3. An employee who is absent due to illness for 5 or more consecutive working days shall be required to submit a medical certificate verifying their inability and/or restrictions to work; all such absences shall be reported to Human Resources by their immediate supervisor;
4. All employee medical documentation shall be retained in the employee’s confidential personnel file in accordance with AP 185 - Records and Retention.
5. No medical documentation, other than medical accommodation plans, shall be retained by the employee’s immediate supervisor; and
6. All employee absences shall be reported using the Division’s automated absence management system.
