AP 541 - Facilities Work Requisition


Small repairs to school facilities are often required that would not usually be considered within larger facilities five year and annual plans.


Service requests must be submitted as a service request on FAME.

1.  Only one Request or repair per service request.

2.  The service request will be reviewed by the Supervisor of Facilities for approval.  Any questions or concerns will be sent by email to the sending school.

3.  Service requests are then distributed to the appropriate Maintenance worker.

4.  Any and all communication regarding repairs should be made in the “NOTES” tab of the Service Request.  Notification of notes and progress are sent to the requestor via email.
