Background
Subject to all existing agreements, the staff of the Division must complete the
AF-405 Leave Request Form in order to actualize an entitled leave or a request for leave.
Procedures
1. The employee completes the Leave Request Form electronically and submits it to the Supervisor.

The principal or supervisor will authorize all leaves for personal
professional development, personal days, EDOs, and medical leaves (three
days or fewer),STF sponsored, CUPE sponsored, First Nation and Metis
sponsored leave
3.
Out of province personal professional development requires approval
from the direct supervisor and the Superintendent of Schools. Requests
for out of province travel from Central Office Personnel requires the
approval of their direct supervisor and either the Deputy Director of
Accountability and Learning Supports, or the Chief Financial Officer
(CFO). ~ Requests must be submitted electronically.
5.
After attending a Personal Professional Development activity the
applicant will forward to the Superintendent of Human Resources, the
following:
5.3 Scanned copy of receipts for all expenses.

After attending a Division Requested Professional Development
activity, the applicant will forward to the sending Superintendent via
the Division mail run or postal service, the following:
6.3 Scanned copy of receipts for all expenses.
6.4 Expense Claims should be submitted electronically within 30 days of the requested leave.
